Please bring the following documents with you to every appointment:

  • Chip card
  • Vaccination card
  • If applicable, routing slip (for employment and termination of employment)
  • Employment examination certificate, if applicable
  • Student ID card, if applicable
  • Current laboratory results/reports of findings/ AHB reports, if applicable
